Accomplish Report of Birth Form and submit in four original copies bearing original signatures and original photos
Requirements to be attached to the Report of Birth Form:
1. Original birth certificate, duly authenticated by the Ministry of Foreign Affairs (MFA) of the country where the child was born and three (3) photocopies;
2. Four (4) photocopies each of the data page of the passports of parents;
3. Proof of residence in country of child’s birth (Visa, Residence Permit or National ID);
4. Four (4) photos of the child-applicant of this Report;
5. Affidavit of late registration for children more than 12 months old (if applicable);
6. One (1) original and three (3) photocopies of marriage certificate of parents;
7. For illegitimate child
• Affidavit of Admission / acknowledgement of paternity for an illegitimate child, ( father will sign);.
• Affidavit to Use Surname of Father (mother will sign);
• Affidavit of Sworn Attestation (mother will sign).
8. The Embassy charges US$25.00 for the Report of Birth, and US$25.00 for notarial of each affidavit. If you opt to pay through bank, please note that you will also need to pay all applicable bank charges.
9. Documents can be sent via courier. Applicant will be responsible for arranging return courier service for applicant’s copy of the Report of Birth.
10. All applications made through a third party, should be accompanied by an authorization letter signed by the applicant.
Note: Documents not in English must be accompanied by an English translation.
