Requirements:
- Personal Appearance of the applicant;
- 1 original and 1 photocopy of PSA Birth Certificate (if born in the Philippines);
- Affidavit of Renunciation of Philippine Citizenship;
- Valid Philippine passport (original passport will be cancelled and retained at the Embassy)
- Original and 1 photocopy of Certificate of Citizenship to another country (original to be returned to the applicant);
- Processing Fee – Affidavit of Renunciation - $25.00

Procedure:
- Submit the documentary requirements listed above and sign the Affidavit of Renunciation in front of the Consular
- Pay the appropriate processing fee at the Cashier and receive the receipt.
- Receive the duly notarized Affidavit of Renunciation of Philippine Citizenship.
- Philippine Embassy will forward copies of the Affidavit, iincluding supporting documents to the Department of Foreign Affairs (DFA), the Philippine Statistics Authority (PSA), and the Bureau of Immigration.
