REPORT OF DEATH
Accomplish Report of Death Form and submit in four original copies bearing original signatures of the person reporting the death.
Requirements to be attached to the Report of Death Form:
- Original death certificate, duly authenticated by the Ministry of Foreign Affairs (MFA) of the issuing country, and three (3) photocopies;
- Embalming Certificate;
- Certificate of Non-contagious Disease;
- Mortuary Certificate.
- Report of Death fee is US$25.00. If you opt to pay through bank, please note that you will also need to pay all applicable bank charges.
Note: Documents not in English must be accompanied by an English translation.
